During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor. We accept most insurances.
Patient Forms
Please print and fill out the New Patient forms so we can expedite your first visit.
Please arrive early so that we can complete your initial paperwork. Also, please bring the following:
- Patient's insurance card
- List of current prescriptions and/or over-the-counter medication, including dose and frequency
- Information about patient's medical and surgical history
- Recent test results, x-rays, or relevant records
- Photo identification card
Insurance and Payment Information
You are responsible for co-payments or charges that are not covered by your insurance. If you have questions regarding billing or which insurance plans we accept, please call our office. Questions regarding insurance coverage and benefits should be directed to your employer or insurance company.
Effective June 1, 2024, AFACC will enforce a new policy regarding missed or canceled appointments without sufficient notice (minimum 24 hours). These are appointments that could have been utilized to offer care to another patient. As a courtesy to our patients, we make every effort to confirm all appointments by text/email.
We will be introducing a $40 fee for missed/no-show appointments or appointment cancellations with less than 24 hour advance notice.
We understand that circumstances arise which may prevent you from keeping your scheduled appointment, however in an effort to provide the best care to all of our patients, please understand that there are few exceptions to this policy.
